US immigration exam

Dr. Rozentul is an USCIS designated physician authorized to conduct medical examinations of aliens for adjustment of status. Our office offers competitive pricing and conducts Immigration medical exam.

What to expect

On the first day of visit, physical exam, blood tests and urine tests will be done. (The actual tests depend on your age and will be recommended by physician at the time of appointment). We will review your vaccination record and provide the vaccinations required to complete the form I-693.

IMPORTANT: please have foreign vaccination records officially translated into English.

You will receive the completed I-693 form, in a sealed envelope in 2 to 3 days.

Documents that will assist

  • Govt issued photo ID (passport or drivers license)
  • Vaccination record (if available)
  • Most adults 18 years to 64 years require proof of Td/Tdap, MMR vaccine (Measles, mumps, rubella) and Varicella (chicken pox). Flu shot is required in flu season (sept to march)
  • Record of prior TB testing (especially if positive)
  • I-693 application can be downloaded and part I can be filled by the applicant. Do not sign until asked to do so by the civil surgeon. If you have an alien number, it can be filled in.
